چکیده

A 78-year-old man underwent an electrophysiologic procedure because of a 25-year history of recurrent episodes of paroxysmal tachycatdia. A wide QRS complex tachycardia that had a long RP interval wa.s induced by ventricular pacing (Fig. 1). The cycle length of the tachycardia was 540 msec. An atrial extrastitiiulus introduced in the high right atrium during the first half of diastole terminated the tachycaidia (Fig. 2). The tachycardia also was tenninated by ventricular pacing at a cycle length of 340 msec (Fig. 3). What is the tachycardia mechanism?

Long RP interval tachycardia. What is the mechanism?

A 41-year-old man with a history of tachycardia refractory to multiple antiarrhythmic drugs was sent to our institution. His 12-lead electrocardiogram demonstrated incessant narrow QRS complex tachycardia with negative P waves in the inferior leads and long RP interval. Occasionally, the tachycardia terminated after a P wave and then was restarted after a sinusal beat. An EP study was performed...

Clinical significance of wide QRS complexes at the termination of paroxysmal supraventricular tachycardias.

BACKGROUND A wide QRS complex is not a rare electrocardiographic phenomenon at the termination of paroxysmal supraventricular tachycardia (PSVT), but no plausible underlying mechanism has yet been proposed. The purpose of the present study was to elucidate the frequency and the underlying mechanism of the wide QRS complexes at the termination of PSVT. METHODS We retrospectively reviewed 305 e...

A Case of Left Bundle Branch Block-shaped wide QRS Complex Tachycardia with diagnostic Ambiguity on a Surface Electrocardiogram

In cases of tachycardia with a broad QRS complex, it is important to differentiate between supraventricular tachycardia (SVT) and ventricular tachycardia (VT). Electrocardiogram (ECG)-based differential diagnoses include VT vs. SVT with aberrant conduction, pre-existing bundle branch block (BBB), intraventricular conduction disturbances, and pre-excitation. Several criteria have been described ...

Wide QRS complex tachycardia in a patient with Wolff-Parkinson-White syndrome and cardiomyopathy: what is the mechanism?

A 32-year old woman with severe mitral regurgitation. congestive heart failure, and Wolff-ParkinsonWhite syndrome developed frequent, nonsustained episodes of a wide QRS complex tachycardia several days postpartum. Figure 1 shows an ECG with telemetry lead I and modified chest lead (MCL) recorded while the patient complained ot" palpitations. The same pattern of initiation and termination of th...
